Debt Review Assessment Process

1. Download the Assessment Form.
2. Kindly complete and sign the Form.
3. Email the form to applications@debtsolutionsdc.co.za with the Required documents.
Required documents:
• A copy of your ID
• Copy of your spouse’s ID (if married in community of property)
• Your last months’ salary advice
• The last salary advice of your spouse (if married in community of property)
• Available statements for the accounts listed in “Part 4 – Debt Obligations.”
4. One of our debt counsellors will contact you to confirm receipt of your application and to initiate the process.
