Debt Review Assessment

How the assessment works

The whole process takes place by email. Please complete the form as accurately as possible — the more complete your information, the sooner we can advise you.

1. Download the Assessment Form

   Save the PDF Assessment Form to your computer or phone.

2. Complete and sign the form

   Fill in every section, including Part 4 – Debt Obligations, and sign where indicated.

3. Email it to us with your documents

   Send the signed form to applications@debtsolutionsdc.co.za together with the required documents listed alongside.

4. We contact you

   A debt counsellor will confirm receipt of your application, talk you through the outcome of your assessment and start the process with you.

Required documents

Please attach the following to your email:

• A copy of your ID
• A copy of your spouse’s ID, if you are married in community of property
• Your latest salary advice
• Your spouse’s latest salary advice, if you are married in community of property
• Available statements for the accounts listed in Part 4 – Debt Obligations
